Option Care Health, Inc. is hiring a Clinical Transition Specialist in Columbus, OH. This role leverages sales techniques to educate patients and families about home infusion services and ensures proper placement for discharge to an alternative site.

The ideal candidate is an RN or Pharmacist with strong relationship-building, customer service, and presentation skills. Travel to care facilities and hospitals is required, with a focus on growing referral sources and meeting sales goals.

Qualifications

• Experience establishing and maintaining relationships at all organizational levels
• Experience providing customer service to internal and external customers
• Basic PC skills: internet, email, MS Word/Excel/PowerPoint
• Ability to plan, organize and make presentations

Responsabilités

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Serves as key point of contact to educate patients, families, and facility staff about services.
• Conducts assessments to ensure patients are viable candidates for home or alternate site care.
• Uses sales techniques to promote services and coordinates with internal partners for smooth transitions.
• Provides hands-on education to patients and caregivers to support timely discharge and independence.
• Communicates with agency staff, medical teams, patients, and families throughout discharge process.
• Collects and reviews pre-admission information and secures sign-offs.
• Partners with Account Manager to execute area business and growth plans.
• Reviews medical records for history and payor information for registration.
• Initiates care transition coordination with referral sources and internal teams.
