The Clinical Transition Specialist role focuses on educating patients and care teams about how services will be facilitated at an alternative site and ensuring proper placement for home health care. As an experienced sales professional with healthcare knowledge, you will build relationships with referral sources, collaborate with internal and external

Qualifications

• Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors.
• Experience applying knowledge of standard practices for all services offered as well as current relevant standards (i.e. ACHC, URAC standards).
• Experience providing customer service to internal and external customers, including meeting quality standards of services, and evaluation of customer satisfaction.

Responsibilities

• Grow referrals and educate discharge staff about services.
• Assess patients to determine suitability for home care.
• Educate patients and families about services.
• Coordinate with hospital staff and internal partners for discharge planning.
• Support transitions to home care and ATS.
• Collaborate with Account Manager to execute growth plans.
