Responsibilities

Ensure coordination of home health services according to standards and procedures. Review and approve patient information during care transitions. Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.

Qualifications

• Graduate of an approved nursing school
• Licensed in the practicing state or compliant with nurse licensure compact rules
• At least two years of nursing experience
• One year in home health or hospice
• Knowledge of relevant laws and regulations
• Basic technology skills, including mobile device operation

Preferred Education and Experience

• Management experience is preferred

Requirements

• Valid state driver’s license
• Dependable transportation and automobile insurance
• Ability to drive safely in all weather conditions
