Job summary

AccentCare, Inc. is seeking a Hospice Registered Nurse Team Director in Miami, FL. This full-time role requires 5+ years of RN experience with at least 2 years in a nursing supervisory position.

Bilingual preferred and driving to multiple locations may be required. Responsibilities include supervising care teams, coordinating scheduling and payroll, ensuring timely assessments, and guiding IDG care plans. The role emphasizes quality care, collaboration with site leadership, and ongoing staff

Qualifications

• Associate degree preferred
• Certified Hospice Nurse (CHPN) preferred
• Three+ years' experience in hospice, home health, general medical surgery, oncology, or geriatrics/pediatrics
• Minimum of one year of supervisory experience
• Possess and maintain valid CPR certification (preferred)
• Must be a licensed driver who can travel to all business locations
• Meet state regulations for program services

Responsibilities

• Supervise direct patient and family care team members with program leadership
• Handle scheduling, payroll, and other administrative duties
• Ensure initial assessments and required reassessments are completed on time
• Oversee IDG development and care plan implementation
• Mentor staff in patient care activities, productivity and documentation
• Support staff education and participate in quality improvement activities

If you meet these qualifications, we would love to meet you:

• Associate degree preferred
• Certified Hospice Nurse (CHPN) preferred
• Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)
• Minimum of one year of supervisory experience
• Possess and maintain valid CPR certification while employed in a clinical role (preferred)
• Must be a licensed driver who can travel to all business locations
• Meet the regulations and requirements of the state(s) in which the program provides services

Responsibilities

As a Hospice Registered Nurse Team Director, you will:

• Supervise direct patient and family care team members, in collaboration with program site leadership
• Responsible for scheduling, payroll, and other administrative duties
• Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization
• Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdisciplinary plan of care for each patient and family
• Collaborate with program site leadership members to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
• Participates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
• Collaborate with site leadership to support staff education efforts
• Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls
• Participates in or lead quality improvement activities, including data collection and aggregation as able
• Reinforces and supports all internal and external communications
