Job summary

AccentCare, Inc. in Newark, Delaware, seeks a Registered Nurse Team Director for hospice, full-time, onsite. You will lead the direct care team, coordinate schedules, and ensure quality patient care across locations.

This role requires supervisory hospice/home health experience, an active RN license, and CHPN preferred. You’ll mentor staff, collaborate with site leadership, and contribute to program development and compliance.

Qualifications

  • Associate degree preferred.
  • CHPN certification preferred.
  • 3+ years in hospice, home health, or related nursing.
  • At least 1 year of supervisory experience.
  • CPR certification preferred.
  • Licensed driver who can travel to all locations.

Responsibilities

  • Supervise direct patient and family care team members with site leadership.
  • Schedule payroll and other administrative duties.
  • Ensure initial and follow-up assessments are completed on time.
  • Oversee IDG care plan development and review.
  • Mentor staff in patient care, productivity and documentation.
  • Support staff education and IDG meetings.
  • Contribute to quality improvement and data collection.
  • Support internal and external communications.

Be the Best Hospice Registered Nurse/RN Team Director You Can Be

If you meet these qualifications, we would love to meet you:

  • Associate degree preferred
  • Certified Hospice Nurse (CHPN) preferred
  • Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)
  • Minimum of one year of supervisory experience
  • Possess and maintain valid CPR certification while employed in a clinical role (preferred)
  • Must be a licensed driver who can travel to all business locations
  • Meet the regulations and requirements of the state(s) in which the program provides services

Responsibilities:

As a Hospice Registered Nurse Team Director, you will:

  • Supervise direct patient and family care team members, in collaboration with program site leadership
  • Responsible for scheduling, payroll, and other administrative duties
  • Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization
  • Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdicisiplinary plan of care for each patient and family
  • Collaborate with program site leadership members to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
  • Participates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
  • Collaborate with site leadership to support staff education efforts
  • Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls
  • Participates in or lead quality improvement activities, including data collection and aggregation as able
  • Reinforces and supports all internal and external communications
