Job summary

AccentCare, Inc. is seeking a Registered Nurse Team Director for our hospice program in Newark, Delaware. This full-time role is on-site, with a Monday–Friday schedule and occasional on-call responsibilities.

You will supervise care team members, manage scheduling and payroll, and ensure timely assessments and documentation. As a leader in patient care, you will collaborate with site leaders to mentor staff, drive quality initiatives, and support education efforts across the care team.

Qualifications

  • Associate degree preferred
  • Certified Hospice Nurse (CHPN) preferred
  • Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (geriatrics, pediatric, etc.)
  • Minimum of one year of supervisory experience
  • Must be a licensed driver and able to travel to all business locations
  • Meet regulations and requirements of the states served by the program

Responsibilities

  • Supervise direct patient and family care team members, in collaboration with program site leadership
  • Schedule, payroll, and other administrative duties
  • Ensure initial assessments and required reassessments are conducted within required time frames, with proper documentation
  • Oversee the Interdisciplinary Group (IDG) in care plan development and review
  • Collaborate with site leadership to monitor staff productivity and documentation
  • Support Back to Bedside efforts and Case Management skills
  • Collaborate with site leadership to support staff education
  • Facilitate IDG meetings and provide ongoing IDG support
  • Participate in quality improvement activities including data collection
  • Reinforce internal and external communications

Be the Best Hospice Registered Nurse/RN Team Director You Can Be


If you meet these qualifications, we would love to meet you:



  • Associate degree preferred

  • Certified Hospice Nurse (CHPN) preferred

  • Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)

  • Minimum of one year of supervisory experience

  • Possess and maintain valid CPR certification while employed in a clinical role (preferred)

  • Must be a licensed driver who can travel to all business locations

  • Meet the regulations and requirements of the state(s) in which the program provides services


Responsibilities:


As a Hospice Registered Nurse Team Director, you will:



  • Supervise direct patient and family care team members, in collaboration with program site leadership

  • Responsible for scheduling, payroll, and other administrative duties

  • Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization

  • Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdicisiplinary plan of care for each patient and family

  • Collaborate with program site leadership members to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols

  • Participates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines

  • Collaborate with site leadership to support staff education efforts

  • Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls

  • Participates in or lead quality improvement activities, including data collection and aggregation as able

  • Reinforces and supports all internal and external communications
