Institute on Aging seeks an experienced supervisor for the MSSP program, overseeing RN Case Management and Care Management staff. You will ensure compliant clinical documentation, staff training, and quality care delivery for elderly and disabled clients living at home or in home-like settings.

The role requires nursing leadership, regulatory knowledge, and strong collaboration with leadership, MSW supervisors, and external partners.

Qualifications

• RN licensed and experienced in care management coordination.
• Experience supervising care management staff and interdisciplinary teams.
• Familiarity with Medi-Cal/Medicare and regulatory compliance.

Responsabilités

• Supervise RN Case Management and Care Management staff in MSSP.
• Participate in hiring, annual performance evaluations, and terminations.
• Ensure day-to-day operations, audits, and regulatory compliance.
• Develop and implement staff education, training plans, and supervision.
• Lead care plan conferences and clinical documentation reviews.
• Coordinate program policies, procedures, and contract adherence.
• Mentor new MSSP staff and interns; liaise with CDA and providers.
