Job summary

Enhabit Home Health & Hospice seeks a dedicated nursing professional to lead coordinated home health services in Rhode Island. You will oversee start of care, recertifications and evaluations, while ensuring collaboration between physicians, patients, caregivers and staff.

The role emphasizes in-person supervision, adherence to state regulations, and strong communication with referral sources. CPR-certified RN with 2+ years home health experience preferred.

Qualifications

• Graduate of an approved school of professional nursing.
• Must maintain valid nursing license per state requirements.
• Experience in home health preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance.

Responsibilities

• Coordinate overall home health services for patients.
• Review and approve start of care, recertifications, resumption of care, or evaluations.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and ensure effective communication for care continuity.
