Job summary

Qualifications

• Care coordination and/or case management experience preferred.
• ACLS certification preferred.
• Chronic Disease Self-Management Certification preferred.

Responsibilities

• Lead and coordinate outpatient practice programs within specialty care.
• Develop plans of care in collaboration with practice providers.
• Prioritize appointments to ensure timely delivery of care.
• Maintain continuity of care and liaison between physicians, patients, and clinical teams.
• Provide coordination and support to clinical staff assigned to the medical practice.
• Educate patients and families about wellness issues and treatment plans.
• Coordinate care with community resources for high‑risk patients.
• Participate in team conferences and divisional meetings to maintain clinical competency.
