Rochester Regional Health in Massena, NY seeks a Care Coordinator RN (Per Diem) to support Behavioral Health across Massena, Potsdam and Gouverneur. The role partners with patients, families, and interdisciplinary teams to develop individualized care plans, educate patients on conditions and medications, and coordinate services to improve outcomes.

This per diem RN position offers flexible shifts, requires NY RN licensure and an AAS in Nursing, with BS preferred.

Qualifications

• AAS in Nursing and NYS RN license required.
• BS in Nursing preferred.
• 3–5 years’ experience in clinical or community resource settings; care coordination desirable.

Responsibilities

• Care Coordination – Identify patients’ needs, develop individualized care plans, educate patients, address barriers, and coordinate with community health services.
• Patient Education & Support – Provide tailored education on self-management, medications, and tests; support high-risk patients; lead patient groups.
• Care Delivery – Ensure safe nursing care, triage calls, facilitate team huddles, collaborate with community resources.
• Quality & Compliance – Collect/report patient data; participate in team conferences; engage in quality improvement initiatives.
