Job summary

Team Select Home Care seeks an experienced Clinical Support Manager, a registered nurse who can oversee a group of clinicians and ensure high-quality, patient-centered care. The role includes coordinating care, supervising staff, and maintaining compliance with state health statutes and CDC/OSHA guidelines.

The position emphasizes leadership in staffing, case management, and QAPI, with responsibilities extending to policy development and clinician training.

Qualifications

• Demonstrates excellent problem-solving techniques and abilities.
• Able to ensure compliance with federal and state regulations governing home health agencies.
• Communicates clearly with staff, physicians, and patients to coordinate care.
• Excellent computer skills are required.

Responsibilities

• Develops, supervises, and evaluates clinicians and caregivers to deliver patient-centered clinical care.
• Assists with analysis of adherence to clinical standards and Best Practices.
• Manages staffing, productivity, outcomes, QAPI, and care coordination goals.
• Coordinates patient care services for an assigned group of patients and staff.
• Ensures staff are educated on QAPI and compliance plans and maintains policy standards.
• Oversees assignment of patient and personnel, referrals, and development of individualized care plans.
• Provides documentation and billing-related support from a clinical perspective.
