Job summary

Alpha Home Health and Hospice seeks a proactive RN Clinical Nurse to join its interdisciplinary team, delivering individualized and safe patient care. You will conduct comprehensive assessments and collaborate with families to achieve optimal outcomes in a patient-centered environment.

Responsibilities include evidence-based practice, mentoring new staff, and participating in protocol development and quality initiatives. A valid RN license and BLS are required; BSN is preferred.

Qualifications

• Minimum of an Associates or Diploma in Nursing; Bachelor's preferred.
• Must hold an active RN license (single or multi-state) and BLS certification.
• Less than one year of clinical experience required; prior direct patient care preferred.
• Strong analytical, critical thinking, communication, teamwork, and ethical standards are essential for success.

Responsibilities

• Perform nursing processes including assessment, diagnosis, planning, implementation, and evaluation.
• Deliver relationship-based, patient-centered care tailored to diverse populations, partnering with patients and families to achieve optimal outcomes.
• Coordinate care through effective communication, documentation, and collaboration, supporting performance improvement and shared governance initiatives.
• Promote evidence-based practice, nursing research, and professional development by serving as a preceptor or mentor.
• Participate in protocol development, data collection, and resource management to optimize patient outcomes and system efficiency.
• Ensure compliance with relevant policies, standards, and ethical guidelines.
