Position Overview
We are seeking a Patient Care Manager to oversee patient care activities, ensure quality service, and lead our care team. Responsibilities include managing visit schedules, responding to inquiries, and facilitating interdisciplinary team meetings.

• Must have hospice case management experience.

You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager, ensuring compliance with all regulations and company policies.

Qualifications:

• Minimum three years of direct patient care experience and two years in hospice or home-based care.
• Licensed Registered Nurse in your state; bachelor’s degree preferred.
• Knowledge of hospice principles, regulations, and best practices.
• Ability to advocate for patients from diverse backgrounds.
