About the Role

We are seeking a Patient Care Manager to oversee and manage our hospice care program. You will be responsible for maintaining high-quality patient care, managing staff, ensuring compliance with regulations, and collaborating with the interdisciplinary team. This role reports directly to the Executive Director, Administrator, or Senior Patient Care Manager.

Responsibilities

• Manage and oversee patient care activities, including scheduling and prioritizing visits based on clinical judgment.
• Respond to phone referrals and inquiries professionally and compassionately, triaging medical concerns within scope of practice.
• Prepare for and facilitate Interdisciplinary Group meetings, including presiding over meetings, presenting patient cases, and coordinating care plans.

Qualifications

• At least three years of direct patient care experience and two years in hospice or home-based care.
• Graduate of an accredited nursing program; bachelor’s degree preferred.
• Current RN licensure in the state of residence.
• Knowledge of hospice principles, regulations (Medicare, Medicaid, JCAHO, ACHC), and best practices.
• Ability to provide culturally competent care to patients from diverse backgrounds.
