Role Overview

We are seeking a Patient Care Manager to oversee our hospice program, ensuring high-quality patient care, staff development, and compliance with regulations. You will report to the Executive Director, Administrator, or Senior Patient Care Manager.

Responsibilities
  1. Manage and oversee patient care activities, including rescheduled, declined, missed, and reassigned visits, prioritizing based on clinical judgment.
  2. Respond promptly and compassionately to phone referrals and inquiries, including triage of medical concerns within scope of practice.
  3. Prepare for and facilitate Interdisciplinary Group (IDG) meetings, including presiding over meetings, reviewing patient charts, and coordinating care team input.
Qualifications
  • Minimum three years of direct patient care experience and two years in hospice or home-based care.
  • Graduate of an accredited nursing school; bachelor’s degree preferred.
  • Current RN licensure in the state of residence.
  • Knowledge of hospice principles, regulations (Medicare, Medicaid, JCAHO, ACHC), and terminal illness care.
  • Ability to serve patients from diverse socioeconomic backgrounds with appropriate advocacy.
