Position Overview

We are seeking a Patient Care Manager to oversee our hospice program, ensuring the delivery of high-quality patient care. Responsibilities include managing patient care activities, teaching and evaluating staff, and ensuring compliance with policies and regulations.

Responsibilities

1. Oversee and manage visit requests, prioritizing and staffing appropriately based on clinical judgment.
2. Respond promptly and compassionately to phone referrals and inquiries, including triage within scope of practice.
3. Prepare for and facilitate interdisciplinary team meetings, review patient charts, and coordinate care plans.

Qualifications

• Minimum three years of direct patient care experience, with at least two years in hospice or home care.
• Graduate of an accredited nursing school; bachelor’s degree preferred.
• Current RN licensure in the state of residence.
• Knowledge of hospice principles, regulations, and best practices.
• Ability to advocate for patients and families from diverse backgrounds.
