Job summary

Muir Home Health in Contra Costa County, CA seeks a Clinical Manager to coordinate patient care and supervise clinical staff, ensuring services align with each physician’s plan and the patient’s needs.

The role requires active licensure in CA (RN/MD/PT/SLP/OT/SW), prior home health leadership, a valid driver’s license, CPR certification, and a commitment to interdisciplinary teamwork. You will lead care planning, case reviews, staff assignments, and participate in quality improvement initiatives.

Qualifications

• Must hold active licensure in California for one of the listed professions.
• Experience in home health management or supervision preferred.
• Valid driver’s license and reliable transportation required.
• CPR certification required.

Responsibilities

• Develop and initiate the plan of care and assume responsibility for ongoing interdisciplinary assessment and development of the individualized plan of care.
• Ensure a Clinical Manager/Supervisor is available during all operating hours.
• Review and update the individualized plan of care in coordination with physicians and the patient/caregiver.
• Assign home care personnel to cases and review patient information and needs.
• Lead case conferences and coordinate with Attending Physicians regarding eligibility for services.
• Assist in staff recruitment, orientation, and participation in quality improvement activities.
