Job summary

KingRecruitingServices is seeking a Patient Care Manager (RN) to lead our home health team in California. You will guide clinical staff, ensure high-quality patient care, and promote a culture of safety, compliance, and compassionate service.

In this role you will supervise staff, coordinate care plans, and drive continuous improvement while maintaining Medicare and state regulatory standards. Strong leadership and clinical expertise are essential to improve patient outcomes and staff

Qualifications

• Must hold an active California RN license.
• Minimum 5 years RN experience, preferably in home care.
• 2 years management or supervisory experience in home health preferred.
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing workplace safety protocols.
