Job summary

• Assess, plan, implement and evaluate patient care for individuals and families.
• Communicate plans using SBAR; participate in handoffs and rounds.
• Initiate discharge planning and coordinate referrals as needed.
• Maintain safety and infection control; anticipate emergencies and respond appropriately.
