Job summary

Emblem Home Health in Arizona is seeking an experienced RN to serve as the Clinical Intake Coordinator, managing patient intake, referrals, and admissions to ensure timely admissions and a seamless transition into home health.

The role requires an active AZ RN license, payer/insurance verification knowledge, and strong communication with patients and providers. This is a full-time position with a Friday–Monday, four 10-hour shift schedule, and remote weekend options.

Qualifications

• Currently licensed as a Registered Nurse (RN) in Arizona.
• Home Health experience is a plus.
• Experience in patient intake, admissions, or care coordination preferred.
• Excellent communication, customer service, and relationship-building skills.
• Knowledge of healthcare regulations and business operations.
• Current CPR certification.

Responsibilities

• Oversee patient intake, referral coordination, and admissions processes.
• Serve as the first point of contact for patients and referral partners.
• Verify insurance, coordinate care services, and ensure timely admissions.
• Maintain relationships with physicians, hospitals, and referral sources.
• Ensure smooth patient transitions to home care and document metrics.
