Job summary

Hospice of the West is seeking an experienced RN to serve as Clinical Director and lead a diverse team of clinicians in the Phoenix area. You will oversee an interdisciplinary team of RNs, Aides, Social Workers, and Chaplains, ensuring high-quality patient care and adherence to guidelines.

The role involves collaboration with the Program Director, Hospice Physician, and Community Liaisons to drive patient growth, maintain compliance, and foster professional development.

Qualifications

• Bachelor's degree in nursing (preferred)
• RN license in the state of practice
• Minimum 3 years nursing experience, including 1 year direct hospice/home health/oncology care
• 1 year supervisory experience in hospice or home health care preferred
• Competence in crisis intervention, pain and symptom management, and family counseling
• Proficiency with computers and EMR systems
• Valid driver’s license and transportation
• CPR certification if required by state

Responsibilities

• Coordinate and oversee a spectrum of patient services in line with hospice program policies
• Provide guidance and support to an interdisciplinary team of clinicians
• Conduct chart audits to ensure regulatory compliance and accurate medical records
• Develop and deliver orientation and in-service training for nursing and clinical staff
• Evaluate and improve clinical staff performance during probation and annually
• Lead interdisciplinary team meetings to update care plans and foster professional collaboration
