Job summary

Crescent Hospice seeks an experienced RN to serve as Clinical Director in Columbia, guiding an interdisciplinary team of RNs, aides, social workers, and chaplains. This leadership role focuses on patient-centered end-of-life care and adherence to regulatory standards.

You will collaborate with the Program Director, Hospice Physician, and Community Liaisons to drive office excellence, grow patient volumes, and maintain quality through chart audits and staff development.

Qualifications

• Bachelor's degree in nursing or healthcare preferred.
• Registered Nurse with a current state license to practice.
• Minimum of 3 years nursing experience with 1 year in hospice/home health/oncology.
• At least 1 year supervisory experience in hospice or home health care preferred.
• Strong crisis intervention, pain and symptom control, and family counseling skills.

Responsibilities

• Coordinate and oversee a spectrum of patient services in line with hospice policies.
• Provide guidance and support to a team of clinicians.
• Conduct chart audits to ensure regulatory, certification, and ethical care compliance.
• Participate in orientation and in-service training for nursing/clinical staff.
• Evaluate and uplift clinical staff performance during probation and annually.
• Lead interdisciplinary team meetings to refine Care Plans and improve patient/family outcomes.
