Job summary

Moments Hospice is seeking a Hospice RN Clinical Manager/Team Director to lead an interdisciplinary team of nurses, social workers, chaplains, and volunteers. You will ensure regulatory compliance and seamless care coordination with families, physicians, and providers across all patient journeys.

The role emphasizes leadership, policy development, and on-call duties, with a focus on compassionate end-of-life care and collaboration within a growing hospice program.

Qualifications

• RN license with a minimum of 3 years healthcare experience, hospice or related settings preferred.
• Bachelor's degree strongly preferred.
• Leadership or management experience a major plus.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate clear communication between medical staff, families, and care teams.
• Lead interdisciplinary meetings and document patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling and performance reviews; foster a positive work environment.
• Act as central communication hub with families, physicians and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
