Job summary

Moments Hospice is seeking an experienced RN Clinical Manager/Team Director to lead an interdisciplinary care team and ensure holistic hospice services. This role coordinates patient care with nurses, social workers, chaplains, and volunteers, while maintaining regulatory compliance and high-quality standards.

You will oversee staff scheduling, care plans, and policy updates, with on-site leadership and after-hours responsibility.

Qualifications

• RN license with a minimum of 3 years healthcare experience
• Bachelor's degree strongly preferred
• Leadership or management experience a major plus
• Valid driver's license (required)

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more
• Ensure adherence to hospice regulations and standards
• Facilitate communication between medical staff and the care team
• Lead interdisciplinary meetings, documenting patient needs and care plans
• Develop and update patient care policies and emergency procedures
• Manage staff scheduling, performance reviews, and foster a positive work environment
• Act as the central hub for communication with families, physicians, and other providers
• Analyze quality metrics and manage a caseload of patients as needed
• Participate in after-hours and on-call duties to ensure seamless care
• Maximize resource utilization and promote a collaborative work culture
