Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Assess patient status and implement care plans in collaboration with the interdisciplinary team.
2. Coordinate care and educate patients.
3. Administer medications and treatments as directed.
4. Monitor and document patient responses.
5. Mobilize resources for complex cases to enhance patient recovery.
6. Use assessment and diagnostic info to anticipate healthcare needs.
7. Ensure effective handoff communication.
8. Manage staffing and develop care plans proactively.
9. Provide individualized comfort measures based on evidence-based practices.
10. Participate in performance improvement activities.
11. Perform other duties as assigned.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Preferred: Clinical nursing experience in a dialysis setting

Rate: $45.00 - $55.00
