Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Assess patient status and monitor care progression with the interdisciplinary team
2. Coordinate care and educate patients
3. Administer medications and treatments as directed
4. Monitor and document patient responses to treatments
5. Mobilize resources for complex cases to enhance patient participation
6. Integrate assessment data to anticipate healthcare needs
7. Ensure effective handoff communication
8. Adjust staffing as needed based on unit observations
9. Provide individualized comfort measures using evidence-based practices
10. Participate in unit performance improvement activities
11. Perform other related duties as assigned

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification

Rate: $45.00 - $55.00
