Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Collaborate with the interdisciplinary team to assess patient status and monitor care progression.
2. Coordinate care and educate patients.
3. Administer medications and treatments as directed.
4. Monitor and document patient responses.
5. Mobilize resources in complex cases to maximize patient participation.
6. Integrate assessment data with intuition to foresee healthcare needs.
7. Ensure effective handoff communication.
8. Adjust staffing as needed based on unit overview.
9. Provide individualized comfort measures using evidence-based practices.
10. Participate in performance improvement activities.
11. Perform other duties as required.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in dialysis preferred

Rate: $45.00 - $55.00
