Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

• Collaborate with the interdisciplinary team to assess patient status and implement and monitor the progression of care.
• Perform care coordination and patient education.
• Administer medications and treatments as directed by provider/physician staff.
• Monitor patients during treatment and document responses.
• Mobilize resources in complex cases to maximize patient control and participation in recovery.
• Integrate assessment and diagnostic information with intuition to foresee healthcare needs.
• Ensure effective handoff communication, including reports on potential developments and issues.
• Maintain awareness of the entire unit and adjust staffing patterns as needed.
• Provide individualized comfort measures using evidence-based and innovative approaches.
• Participate in performance improvement activities.
• Perform other related duties as required.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in a dialysis setting preferred

Rate: $45.00 - $55.00
