This role acts as a liaison among patients, providers, leadership, and interdisciplinary teams to improve care coordination, regulatory compliance, and performance improvement.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license.
• 2+ years of clinical nursing experience.

Responsibilities

• Coordinates clinical, operational, and quality activities to support patient care services and organizational objectives.
• Serves as a liaison among patients, providers, leadership, and interdisciplinary teams to facilitate effective care coordination, regulatory compliance, and performance improvement initiatives.
