Job summary

Hillcrest HealthCare System is seeking a Clinical Navigator RN to join our Tulsa team on the day shift. The role coordinates patient care across the continuum and serves as a clinical resource for patients, families, and interdisciplinary teams, promoting high-quality outcomes.

Responsibilities include care coordination, patient education, and implementing clinical program initiatives while upholding evidence-based practices and collaborative care.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license to practice in Oklahoma.
• BLS certification obtained within 14 days of hire.

Responsibilities

• Coordinates and facilitates patient care across the continuum for designated populations.
• Acts as a clinical resource, patient advocate, and liaison among patients, families and teams.
• Supports care coordination, patient education, and evidence-based practice initiatives.
• Contributes to improved outcomes through effective communication and collaboration.
