BrioHR Pte Ltd seeks an RN Clinical Coordinator for LifeCircles PACE in Muskegon, MI. The role delivers compassionate nursing care across clinic, home, and care settings, developing individualized care plans and using evidence-based interventions.

You will apply clinical judgment, collaborate with a multidisciplinary team, and support transitions between care environments to ensure high-quality care for participants.

Qualifications

• RN license to practice in Michigan.
• Diploma or degree from an accredited School of Nursing or University.
• Minimum of one year working with frail elderly or long-term care.
• Previous experience in home care or long-term care is preferred.
• Excellent communication and interpersonal skills.

Responsabilités

• Facilitate participant integration into LifeCircles’ healthcare system, including medication reviews and immunizations.
• Conduct initial and periodic assessments in homes, facilities, and care centers.
• Identify participant needs and perform advanced nursing assessments using critical thinking.
• Assess ADL/IADL support needs in the home.
• Document all care in the participant's medical records.
• Coordinate care plans with Home Care Coordinators and IDT.
• Administer treatments, medications, and other prescribed care.
• Participate in on-call nursing shifts.
• Support QA and PI activities.

As an RN Clinical Coordinator, you’ll deliver compassionate, person-centered nursing care across a variety of settings, including our clinic, participants’ homes, and care facilities. You’ll play a vital role in helping participants achieve their personal health goals through individualized care plans, thoughtful assessments, and evidence-based interventions.

Key Responsibilities

• Facilitate participant integration into LifeCircles’ healthcare system, including conducting medication reviews, administering immunizations, and monitoring both acute and chronic medical conditions.
• Conduct initial and periodic assessments in participant homes, acute and long-term care facilities, the PACE center, and other community settings.
• Identify participant needs, set priorities, interpret diagnostic results, and perform advanced nursing assessments using critical thinking.
• Assess clinical and Activities of Daily Living (ADL)/Instrumental Activities of Daily Living (IADL) support needs in the home environment.
• Promptly report unusual findings to the primary care provider, supervisor, and/or Interdisciplinary Team (IDT).
• Actively participate in IDT meetings and contribute to the development of the participant care plan across all care settings.
• Collaborate with Home Care Coordinators to develop and monitor care plans.
• Administer treatments, procedures, medications, and other prescribed care.
• Oversee and monitor participant medications.
• Provide clear explanations of nursing procedures to participants, offer discharge teaching, and coordinate transitions between care settings.
• Coordinate patient care with other healthcare personnel, evaluate instituted care measures, and ensure the 24-hour delivery of care services.
• Work with Home Care Coordinators and other team members to coordinate Durable Medical Equipment (DME) and medical supplies (e.g., oxygen, hospital beds, incontinence supplies, wound care supplies, ostomy supplies, tube feeding supplies).
• Accurately document all care in the participant's medical records.
• Delegate nursing care to appropriate personnel.
• Actively support and participate in Quality Assurance and Performance Improvement (QAPI) processes.
• Participate as part of the on-call nursing staff.

Skills, Knowledge and Expertise

• Current Registered Nurse (RN) license to practice in the State of Michigan.
• Diploma or degree from an accredited School of Nursing or University.
• Minimum of one year of experience working with the frail elderly or long-term care population.
• Previous experience in home care or long-term care is preferred.
• Excellent communication and interpersonal skills, with the ability to work effectively with participants, families, and the healthcare team.
