Job summary

PACE of Southwest Michigan is seeking an RN Clinical Coordinator in St. Joseph, MI. You will perform nursing assessments, care coordination, education, and ongoing clinical management for an assigned panel of participants.

Ideal candidates are RNs with at least 3 years of nursing experience, Michigan licensure, CPR certification, and a passion for improving outcomes in older adults. This exempt, full-time role offers a 40-hour work week, Monday–Friday schedule, and competitive benefits.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Must have three (3) years' experience as a practicing nurse in a health-related area.
  • Licensed Registered Nurse in the State of Michigan.
  • Must possess a valid driver's license.
  • Must be CPR certified.
  • Minimum of one (1) year working with the frail and elderly.

Responsibilities

  • Coordinates nursing care and follow-up for an assigned panel of PACE participants.
  • Completes required nursing assessments.
  • Reviews relevant clinical information, diagnostic results, and outside records using nursing judgment and critical thinking.
  • Develops, reviews, and updates individualized plans of care.
  • Participates in interdisciplinary care planning and clinical discussions.
  • Coordinates and follows up on provider orders, referrals, diagnostic testing, specialist recommendations, medication changes, and other identified care needs.
  • Supports transitions of care following hospitalization, emergency department visits, skilled nursing stays, and other changes in care setting.
  • Provides participant and caregiver education related to disease management, medications, treatment plans, symptom monitoring, and preventive care.
  • Triage and provide appropriate nursing intervention.
  • Maintains complete, timely, and accurate documentation of nursing care and coordination activities.
