Job summary

PACE of Southwest Michigan is seeking an RN Clinical Coordinator in St. Joseph, MI, Monday–Friday, 8am–4:30pm, exempt with a 40-hour week. You will provide nursing assessment, care coordination, and ongoing clinical management for a defined panel of participants.

Responsibilities include care planning, education for participants and caregivers, and coordinating provider orders, referrals, and tests. Prior experience with frail elderly populations is preferred.

Qualifications

• Graduate of an accredited School of Nursing.
• Three years' experience as a practicing nurse in a health-related area.
• Licensed Registered Nurse in the State of Michigan.
• Must possess a valid driver's license.
• CPR certified.
• Minimum of one year working with the frail and elderly.

Responsibilities

• Coordinates nursing care and follow-up for an assigned panel of PACE participants.
• Completes required nursing assessments.
• Reviews relevant clinical information, diagnostic results, and outside records using nursing judgment and critical thinking.
• Develops, reviews, and updates individualized plans of care.
• Participates in interdisciplinary care planning and clinical discussions.
• Coordinates and follows up on provider orders, referrals, diagnostic testing, specialist recommendations, medication changes, and other identified care needs.
• Supports transitions of care following hospitalization, emergency department visits, skilled nursing stays, and other changes in care setting.
• Provides participant and caregiver education related to disease management, medications, treatment plans, symptom monitoring, and preventive care.
• Triage and provide appropriate nursing intervention.
• Maintains complete, timely, and accurate documentation of nursing care and coordination activities.

Skills, Knowledge And Expertise
