Job summary

Trinity Health PACE in West Springfield, MA seeks a Clinic Manager (RN) to oversee nursing services across community, day center, and clinic settings. You will lead operations, coordinate with the primary care team, and drive high-quality, person-centered care for PACE participants.

Requirements include an active RN license in the state, BSN, CPR, and 3+ years in healthcare with geriatrics experience. Strong leadership and communication skills are essential.

Qualifications

• Current, active, and unencumbered RN license in the state.
• Bachelor’s degree in Nursing (BSN) required.
• Active CPR certification.
• Minimum of 3 years of healthcare experience, with emphasis in geriatrics.
• At least 1 year of experience working with frail or elderly populations (or training upon hire).
• Strong leadership, conflict management, and decision-making skills.
• Excellent interpersonal and communication skills, both written and verbal.
• Proficiency in Microsoft Office Suite and familiarity with email/calendaring.
• Independent decision-making ability and high personal accountability.
• Strong organizational and problem-solving skills.
• Ability to prioritize tasks and adapt to changing work priorities.
• Critical thinking and analytical skills for effective decision-making.

Responsibilities

• Attend required training and educational sessions.
• Oversee the PACE clinic as the central hub to meet participant needs.

Position Purpose

The Clinic Manager is responsible for the oversight and delivery of all nursing services provided to participants in the PACE Organization (PO). Services may be delivered in the community, day center, or primary care clinic. This role ensures high-quality care through effective clinic operations and coordination with the primary care team.

Essential Functions

• Lead by example in demonstrating TH PACE values to participants, visitors, volunteers, and colleagues.
• Foster a collaborative culture across all divisions within TH PACE.

Minimum Qualifications

• Current, active, and unencumbered RN license in the state.
• Bachelor’s degree in Nursing (BSN) required.
• Active CPR certification.
• Minimum of 3 years of healthcare experience, with emphasis in geriatrics.
• At least 1 year of experience working with frail or elderly populations (or must receive training upon hire).
• For North Carolina: Must have a signed Employee Medical Statement within the past 12 months.
• Strong leadership, conflict management, and decision-making skills.
• Excellent interpersonal and communication skills, both written and verbal.
• Proficiency in Microsoft Office Suite and familiarity with electronic mail and calendaring systems.
• High level of personal accountability and independent decision-making ability.
• Strong organizational and problem-solving skills.
• Ability to prioritize tasks and adapt to changing work priorities.
• Critical thinking and analytical skills for effective decision-making.
