A Moment's Notice Health Care in Vero Beach, FL is seeking an RN Care Manager to coordinate and oversee private-duty home care for clients in their homes. The role emphasizes relationship-based, client-centered care and collaboration with families and physicians.

The position is part-time with potential to grow into full-time, offering an hourly rate of $40–$45 and opportunities for wage increases. Schedule varies by client needs and caseload, including possible evenings and minimal weekends.

Competenze

• Graduate of an accredited school of nursing.
• Current Florida RN license
• Minimum two years of nursing experience; hospital or geriatric experience preferred
• Valid driver's license with clean driving record
• Reliable, insured vehicle
• CPR certification or willingness to obtain within 90 days of hire
• Strong communication, organization, and problem-solving skills
• Access to email, texting, and electronic documentation tools

As an RN Care Manager, you will play a vital role in assessing client needs, coordinating care, supporting caregivers, advocating for clients, and helping individuals maintain their independence and quality of life.

Position Details

• Vero Beach, FL
• Part-Time with the opportunity to grow into Full-time if desired.
