Job summary

Arcare seeks a Chronic Disease Nurse Supervisor to oversee the Chronic Disease Case Management nurses, providing education, case management, and coordination for remote care services. This role works with clinical leadership to enroll patients and ensure timely paperwork and evaluations.

The position emphasizes program development, quality improvement, and collaboration with community partners to enhance patient outcomes and care efficiency across multiple settings.

Qualifications

• RN license in appropriate state is required.
• At least two years of nursing experience; management experience preferred.
• Ability to learn and adapt to changing technologies and software used by the organization.

Responsibilities

• Oversee CDCM Nurses and supervise Chronic Disease Care Management in assigned clinics.
• Lead program evaluation, performance improvements, and quality initiatives.
• Coordinate with Remote Care team to deliver patient-centered care across settings.
• Ensure paperwork, screenings, and education are current and completed timely.
• Collaborate with leadership, providers, and community partners to improve outcomes.
