Job summary

Arcare seeks a Chronic Disease Nurse Supervisor to oversee CDCM Nurses, coordinate care for primary care and HIV patients, and support Remote Care initiatives. The role includes ensuring timely documentation, education, and patient-centered service delivery across phone, telehealth, and in-person visits.

The supervisor collaborates with senior leadership to optimize program operations, ensure policy compliance, and drive continuous quality improvement in a multi-site care network.

Qualifications

• RN license in appropriate state required.
• At least two years of nursing experience; management experience preferred.
• Proficiency with MS Word/Excel and electronic record keeping systems; ability to learn new software.

Responsibilities

• Supervise Chronic Disease Case Management Nurses and lead the Coordinated Care team.
• Oversee platform and vendor selections and staff education materials.
• Develop and direct annual program evaluation and quality improvement activities.
