Qualifications

Responsibilities

• Support patient assessment and triage prioritization (walk-in/phone).
• Execute protocol-driven chronic condition management.
• Perform nursing procedures including wound care and meds.
• Educate patients and caregivers on chronic conditions.
• Review medications during nurse-led visits.
• Conduct onsite testing and identify gaps in preventative screenings.
• Review and act on high-priority abnormal labs.
• Collaborate with providers on complex care plans.
• Use protocols for med management and renewals.
• Delegate tasks to medical assistants and phlebotomists.
