You will perform triage, chronic condition management, wound care, medication administration, patient education, and collaboration with providers to coordinate complex care plans. EMR experience and BLS/CPR are required.

Qualifications

• Active, unrestricted registered nurse licensure in the state of residence.
• 2+ years of healthcare experience; RN preferred.
• BLS or CPR certification.
• EMR experience required.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and medication administration.
• Provide comprehensive patient and caregiver education regarding chronic conditions.
• Conduct medication reviews within nurse-led visits.
• Conduct onsite testing and use patient data to identify gaps in preventative screenings.
• Review and act upon high-priority abnormal laboratory results.
• Partner with providers to manage complex care plans and coordinate transitions.
• Utilize standardized protocols for medication management and prescription renewals.
• Delegate tasks to medical assistants and phlebotomists as needed.
