Responsibilities

• Perform wound care and medication administration.
• Educate patients and caregivers about chronic conditions.
• Conduct medication reviews during nurse-led visits.
• Perform onsite testing and monitor preventative screenings.
• Review abnormal lab results and coordinate with providers.
• Coordinate transitions of care and delegate tasks to staff.
• Use standardized protocols for med management and renewals.
