Qualifications

• Active, unrestricted RN licensure in your state.
• 2+ years healthcare experience as RN preferred.
• BLS or CPR certification required.
• EMR experience required.

Responsibilities

• Support patient assessment and triage for walk-ins and calls.
• Manage chronic conditions using protocol-driven care.
• Perform wound care and medication administration.
• Educate patients and caregivers on chronic conditions and prevention.
• Review medications during nurse-led visits to ensure safety.
• Conduct onsite testing and analyze data to close screening gaps.
• Review high-priority abnormal labs promptly.
• Collaborate with providers and the care team on complex plans.
• Use standardized protocols for med management and renewals.
• Delegate tasks to medical assistants and phlebotomists to maintain flow.
