Job summary

Southern Health Partners, located in Robertson County Jail, TN, is seeking an RN Chronic Care Nurse to manage ongoing health conditions in a correctional setting. The role emphasizes compassionate, structured care within a secure environment and involves coordinating with a professional medical team.

You'll work Monday–Friday, 8am–4:30pm, with a focus on chronic care plans, patient monitoring, and medication support to maintain patient safety and continuity of care.

Qualifications

• RN license required.
• Experience in chronic care management preferred.
• Ability to coordinate with healthcare providers.

Responsibilities

• Develop, implement, and evaluate chronic care plans.
• Monitor stability and progression of chronic conditions.
• Conduct chronic care follow-ups and clinics per established protocols.
• Track key health indicators (BP, glucose, labs, mental health).
• Identify at-risk patients and intervene proactively.
• Coordinate with internal and external providers for continuity of care.
• Support MAT medication pass and ensure patient safety when staff are occupied.

Location: Robertson County Jail, TN

Position: RN – Chronic Care Nurse

Scheduled: Monday - Friday (8am to 4:30pm)

QualificationsChronic Care Management

• Develop, implement, and evaluate individualized chronic care plans
• Monitor patients with chronic conditions for stability, compliance, and progression
• Conduct chronic care follow-ups and clinics according to established protocols
• Track and document key health indicators, including blood pressure, glucose levels, labs, and mental health status
• Identify patients at risk for complications and intervene proactively
• Promote continuity of care by coordinating with internal and external healthcare providers
• Support Medication-Assisted Treatment (MAT) medication pass by administering and/or assisting with controlled substances requiring enhanced monitoring, helping ensure patient safety and compliance—especially when primary nursing staff are occupied with routine medication administration
