Henry Community Health in New Castle, IN seeks an RN Care Coordinator to collaborate with providers and the primary care team to assess readiness and develop patient-centered care plans for chronic conditions.

You will guide patients through the health care system, coordinate care with specialists, and support transitions in care while tracking progress and outcomes for improved health. Indiana RN licensure required, some travel may be needed.

Formación

• Current Indiana RN licensure required.
• Previous experience in caring for chronic disease patients.
• BSN and 2+ years of experience preferred.
• CCM (certified case manager) preferred.
• Demonstrates evidence of leadership, communication, education, collaboration and counseling skills.
• Outstanding communication skills, both written and verbal.
• Excellent organizational skills
• Ability to travel locally.
• Knowledge of communication strategies for accessibility and adaptability when required.

Responsabilidades

• Collaborate with providers and care team to set goals and action steps.
• Develop patient-centered care plans for chronic conditions.
• Coordinate access to medical and specialty services.
• Facilitate transitions in care and referrals.
• Track results, medications, and follow-up adherence.
• Link patients to social services and transportation.
• Conduct regular follow-ups to evaluate progress and health outcomes.
• Act as the primary contact for patient, family, payers, and team.
