Job summary

LiveWell Homecare Agency in Grovetown, GA is seeking a registered nurse to assess, plan, implement, and evaluate patient care in collaboration with the care team. You will serve as the primary liaison for patients, families, providers, and staff, ensuring patient-centered care and satisfaction.

The role requires an Associate or Diploma in Nursing, a current RN license (multi-state compact or single state), and BLS certification upon hire.

Responsibilities

• Assess, plan, implement, and evaluate care plans in collaboration with the care team.
• Serve as a primary liaison between patients, families, providers, and staff to ensure patient-centered care and satisfaction.
• Monitor patient conditions, document assessments and interventions, and communicate changes effectively.
• Advocate for patients’ needs and support education for patients and families on health issues.
• Participate in performance improvement initiatives and contribute to a positive work environment.
• Maintain professional competence through ongoing education and involvement in professional organizations.
• Mentor fellow nurses, promote shared governance, and adhere to safety, confidentiality, and professional standards.
