Job summary

Signature HealthCARE of Erin is seeking an experienced Registered Nurse to provide direct resident care, including medications and wound care, while supervising nursing staff. The role supports a rehab to home program and adheres to long-term care regulations.

The ideal candidate holds an active RN license, with 1+ year clinical nursing experience and familiarity with EHR systems. Strong communication and independent work skills are essential for success in this role.

Qualifications

Current, active RN license in the state of practice.
CPR/BLS certification preferred (American Heart Association or American Red Cross).
Minimum one year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.
Effective verbal and written communication skills.
Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
Familiarity with electronic health records (EHR) systems.

Responsibilities

Provide direct nursing care to residents, including medication administration and wound care.
Assist residents with activities of daily living (ADLs) with dignity and safety.
Supervise nursing assistants and medication aides on shift.
Monitor residents’ condition and communicate changes to the physician.
Develop and review individualized resident care plans with the care team.
Complete accurate clinical documentation per policy.
Coordinate admissions, transfers, and discharges.
Participate in quality assurance activities and staff education.
Perform other duties as assigned by the Director of Nursing.
