Job summary

Signature HealthCARE of Erin is seeking an experienced RN to join our team and provide direct nursing care within a 164-bed facility with a strong therapy program. You will supervise staff, coordinate care plans, and ensure regulatory compliance while delivering person-centered care.

You will work collaboratively with a multidisciplinary team to support residents, manage medications, and document care accurately. This role offers competitive pay and a comprehensive benefits package.

Qualifications

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with Federal and State long-term care regulations and EHR systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
