Job summary

Vighter is seeking a Registered Nurse (RN) Case Manager to provide primary case management for the Wounded Warrior program at Walter Reed National Military Medical Center in Bethesda, MD. The role focuses on inpatient and outpatient care coordination, discharge planning, and collaboration with military and civilian healthcare teams.

Qualified candidates will hold an ADN or BSN with 5+ years of nursing experience, including case management.

Qualifications

• Minimum five years of nursing experience with full-time case management experience; for ADN three years, for BSN two years.
• Active, valid, current RN license in any U.S. state.
• Current BLS and ACLS certifications.
• CCM or ANCC certification or eligibility; obtain within 12 months.

Responsibilities

• Provide case management, care coordination, and discharge/disposition planning for inpatient and outpatient care.
• Identify and assess patients for case management (DES, wounded, ill, or injured Service Members).
• Develop patient-specific care plans with patients, families, and healthcare team.
