Qualifications

• Bachelor’s degree in Nursing from ACEN/CCNE accredited program.
• At least 24 months of full-time case management or care coordination experience.
• CCM or ANCC certification eligibility.
• Current BLS/Healthcare Provider certification.
• Ability to obtain government background clearance.

Responsibilities

• Identify candidates for CM/care coordination using analytics and CM registries.
• Provide CM for Wounded Warrior population and other patients as needed.
• Develop personalized care plans and facilitate access to healthcare services.
• Educate patients and families and advocate for timely care.
• Document encounters in EHR per DHA/MHS guidance.
• Coordinate with PCM teams and provide updates on outcomes.
• Provide CM training to MTF personnel.
• Participate in MTF meetings and DHA/network webinars.
