Job summary

Maxwell Air Force Base seeks a Registered Nurse - Wounded Warrior Case Manager to support active duty wounded, ill, and injured personnel. You will identify candidates for CM, create personalized care plans, coordinate access to services, and advocate for timely care within DHA guidelines.

You will work with PCM teams, provide BH CM services where needed, and document all interactions in the EHR. A nursing license, BLS, and the ability to obtain security clearance are required.

Qualifications

• Bachelor's degree in Nursing from ACEN/CCNE accredited program.
• 2+ years of post-graduate RN experience in case management or care coordination.
• Active, unrestricted RN license; BLS certification.
• Ability to pass government background check/security clearance.

Responsibilities

• Identify patients who would benefit from CM services and notify PCM teams.
• Provide CM and care coordination to patients with personalized care plans.
• Collaborate with PCM teams and provide regular updates and outcomes.
• Deliver BH CM services in collaboration with Behavioral Health Lead.
• Document CM encounters in the patient's EHR per DHA guidance.
• Maintain registry of CM patients and coordinate transitions in/out of CM.
