• BSN degree preferred.
• Minimum 2 years cardiovascular nursing clinical experience; case management experience preferred.

• Maximizes positive financial outcomes for designated case types.
• Develops organizational skills and effective time management on a daily basis.
• Assesses efficiency and cost effectiveness of interdepartmental systems.
• Communicates effectively with patients, families, physicians and staff.
• Partners with CM leadership to plan and improve care delivery and utilization processes.
